
RISK ASSESSMENT FOR ATHLETES
The purpose of this questionnaire is to identify if you are at-risk for or experienced symptoms of Relative Energy Deficiency in Sport (REDs). REDs can negatively impact overall health and athletic performance.
Have you experienced:

A plateau or decrease in athletic performance, taking longer times to recover from workouts or feeling more fatigue than usual?

Sports related injury/ difficulty healing from injury. More frequent illnesses or taking longer to recover from illness?

Increased irritability, anxiety or mood changes?

Fatigue more than usual? Is it harder to concentrate?

Any of the following digestive symptoms: increased bloating, gas, constipation, stomach ache?

Any change in your menstrual cycle? (Length of cycle, missed periods, lighter periods) (If applicable)

Has your activity level increased recently? (In athletics or outside of)

Are you trying to lose weight or change your body shape? Have you experienced any changes in weight?

Have you been limiting intake of any foods? Have you had any changes in your eating patterns recently including for food sensitivity/allergies?

Do you feel any stress around food choices? Do you have more thoughts about food and eating more than usual?
Answering YES to 1 or more statement indicates further assessment.
